Nebraska FH-41 Report of Visual Evaluation (School)

Nebraska requires a visual evaluation for students entering school at the beginner grade, kindergarten or first grade, and for transfers from out of state into any grade (Neb. Rev. Stat. 79-214). This convenience form lets the examiner certify the evaluation and report pass or fail results, distance and near acuity, and any recommendation for further evaluation.

Who completes it

The examiner (physician, PA, APRN or optometrist) completes Part I with the results and signs it. The parent or guardian completes Part II, consenting to release the information to the named school.

Submitting and payment

How to submit it
The completed form is returned to the school health office of the school named in the parent's consent.
Who pays
The form sets no fee and does not address payment for the visual evaluation.

Good to know

  • Five items are marked pass, fail or recommend further evaluation: amblyopia, strabismus, internal eye health, external eye health and visual acuity.
  • Acuity is recorded for each eye at 20 feet and at 16 inches, noting whether the child was tested with or without glasses.
  • The form states that no specific form is required by statute; it is offered as a convenience to families and the child's health care provider.
  • Nebraska DHHS also posts FH-40, a waiver of the physical exam and visual evaluation requirement; the current FH-41 is dated 4/2025.
