Frequently asked questions

Which Nebraska students need a school physical?

Under Neb. Rev. Stat. 79-214, a physical examination is required for entry at the beginner grade (kindergarten or first grade), for seventh grade, and for students transferring from out of state. FH-42 is a convenience form for reporting it.

Can Nebraska FH-42 be used as a sports physical?

The form includes a checkbox certifying that the student passed the physical examination and is physically able to participate in interscholastic athletics, plus a line for activities the student should not participate in and significant chronic health concerns.

Is a shorter form available instead of FH-42?

Yes. Nebraska DHHS also posts FH-43, Certification of Physical Examination, a short certificate stating that the required exam was done. FH-42 is the full report, and the form notes that no specific form is required by statute.
