Iowa Certificate of Dental Screening (HHS)

Iowa requires children newly enrolling in kindergarten or ninth grade to submit a Certificate of Dental Screening to their school, and Iowa HHS says this form is the only acceptable one. The provider records the screening date and one treatment need: no obvious problems, requires dental care, or requires urgent dental care. The certificate is invalid unless every field is complete.

Who completes it

The screening provider completes the screening section and signs with credentials: for kindergarten, a dentist, dental hygienist, physician, nurse, ARNP or PA; for ninth grade, a dentist or dental hygienist only.

Good to know

  • A kindergarten screening is valid from age 3 until four months after enrollment; a ninth-grade screening from one year before until four months after enrollment.
  • Check treatment needs based on screening results before any treatment is provided; the form defines tooth decay, white spot lesions and gum infection.
  • An authorized provider may act as recorder, transferring results from another health document, which must be attached to the certificate.
  • Screenings by out-of-state providers are allowed, and a separate Certificate of Dental Screening Exemption is also posted by Iowa HHS.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can do the Iowa ninth-grade dental screening?

Only a licensed dentist or dental hygienist. For kindergarten, a dentist, dental hygienist, physician, nurse, advanced registered nurse practitioner or physician assistant may screen. Out-of-state providers are allowed, and the provider checks one credential box on the certificate.

Does the Iowa dental screening replace a dental exam?

No. The certificate states that a screening does not replace an exam by a dentist and that children should have a complete examination by a dentist at least once a year. Regional I-Smile coordinators help students who need a screening or follow-up care.
