Iowa 470-5152 Child Care Provider Physical Exam

Iowa requires child care center personnel and child development home providers to have a physical examination covering immunizations, tuberculosis screening and overall health, and form 470-5152 records it. The examiner reviews immunizations against the ACIP schedule, completes a baseline TB symptom and risk factor screen, and decides whether the person may be involved with child care.

Who completes it

An MD, DO, PA, ARNP or DC completes the whole report, circles the provider type and signs; a stamp may be used.

Submitting and payment

How to submit it
Give the completed report to the employee or provider for the center or child development home's file.

Good to know

  • Only perform a TST or IGRA if the person has an identified risk factor or current symptoms of TB, and do not test people with past positive results.
  • If the person declines recommended vaccines after consultation, the examiner lists the declined vaccinations instead of certifying them as current.
  • The examiner also answers whether the person has a condition that limits their ability to safely supervise or evacuate multiple dependent children in an emergency.
  • A Spanish version, 470-5152(S), is posted alongside the English edition (Rev. 01/23).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does Iowa require a TB test for child care staff?

Not always. All child care staff and providers need a baseline TB screening made up of a symptom assessment and a risk factor screen. The form says to perform a TST or IGRA only if there is an identified risk factor or current symptoms, and not to test people with previous positive results.

What conclusions can be marked on Iowa form 470-5152?

Three: the individual may be involved with child care; may be involved with child care with accommodations and restrictions, which are described on the form; or may not be involved with child care.
