Maryland School Medication Administration Authorization

Maryland public schools can only give a student medication when this form is completed in full by the prescriber and parent. Clinicians receive it whenever a student needs a daily, PRN or emergency medicine at school; the order is valid for one year from the prescriber's signature and is reviewed and signed by the school nurse.

Who completes it

The prescriber completes the Prescriber's Authorization and, when appropriate, signs the self-carry authorization for emergency medication. The parent signs the consent, and the school RN records verbal orders, approves self-carry and reviews the order.

Submitting and payment

How to submit it
The completed form goes to the school nurse, and an adult must bring the medication to school in a properly labeled container.
Who pays
The form sets no fee and does not address payment for completing the order.

Good to know

  • A new form is needed annually, for each medication, and whenever the dosage or time of administration changes.
  • The order lists condition, medication, dose, route, time or frequency, PRN frequency and symptoms, relevant side effects and start and end dates, with an original signature or signature stamp only.
  • Self-carry or self-administration of emergency medication must be authorized by the prescriber and approved by the school nurse under the State medication policy.
  • At the end of the school year an adult must pick up any remaining medication, otherwise it will be discarded; the current printing is dated August 2025.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is a Maryland school medication order valid?

The form states the order is valid for one year from the date of the prescriber's signature. A new form is required annually, for each medication, or when there is a change in dosage or time of administration.

Can a student carry an inhaler or epinephrine at a Maryland school?

Self-carry or self-administration of emergency medication may be authorized by the prescriber in a dedicated section, but it must also be approved by the school nurse according to the State medication policy. Both signatures are dated on the form.

Will the school nurse contact the prescriber?

The form tells families the school nurse (RN) will call the prescriber, as allowed by HIPAA, if a question arises about the child or the medication. There is also a line for the nurse to record a verbal order taken for the medication.
