Maryland OCC 1216 Child Care Medication Authorization

Maryland child care providers may only give a child prescription or over-the-counter medication when this form is completed fully. The prescriber writes the order, the parent consents and confirms a first dose was given at home without problems, and staff log every dose on the second page. Asthma, allergy and seizure medicines use OCC 1216A, 1216B and 1216C instead.

Who completes it

The prescriber completes the Prescriber's Authorization and signs it. The parent or guardian completes the authorization section, and child care staff complete the staff checklist and the administration log.

Submitting and payment

How to submit it
The completed form is returned to the child care program with the medication in a pharmacy-labeled or original container.
Who pays
The form sets no fee and does not address payment for completing the order.

Good to know

  • The authorization may not exceed one year; the prescriber enters from and to dates and, for PRN medication, the symptoms, how often and for how long.
  • The prescriber's signature must be an original signature or signature stamp, and the form states the parent or guardian cannot sign that line.
  • For school-age children only, the prescriber marks whether the child may self-carry and whether the child may self-administer the medication; the program may revoke that permission.
  • The parent attests they have given at least one dose of the medication without adverse effects before the program begins giving it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does OCC 1216 cover over-the-counter medicine in Maryland child care?

Yes. The form is required for both prescription and non-prescription medications. Prescription medication must be in a container labeled by the pharmacist or prescriber, and over-the-counter medication must be in the original container with the label intact.

How long is a Maryland OCC 1216 authorization valid?

The authorization may not exceed one year. The prescriber enters the dates the medication is to be given, and at the end of the authorized period an authorized individual must pick up the medication or it will be discarded.

When should OCC 1216A, 1216B or 1216C be used instead?

Maryland has separate forms for asthma medication (OCC 1216A, an asthma action plan), allergy and anaphylaxis medication (OCC 1216B) and seizure medication (OCC 1216C). Each states it replaces or is used without OCC 1216 for that medication.
