Maryland OCC 1204 Medical Evaluation for Child Care

Maryland's Office of Child Care asks for a medical evaluation of people who work, volunteer or reside in a child care program. The clinician screens for tuberculosis, discusses immunizations and states whether the person is medically and emotionally fit, then marks the age groups they have the strength and mobility to care for.

Who completes it

The person being evaluated (or a guardian, if a minor) fills in their name and program and signs the release to the Office of Child Care. A physician, registered physician assistant or certified registered nurse practitioner completes and signs the evaluation section.

Submitting and payment

How to submit it
The completed form is returned to the individual for the child care program's licensing file; the clinician adds a stamp or full address and telephone.
Who pays
The form sets no fee and does not address who pays for the evaluation.

Good to know

  • A TB risk and symptom screening is required; a TB test is done only if indicated or required by the Local Health Officer, and the clinician states whether the person is free of communicable TB.
  • If the person is not fit to work, volunteer or reside in a child care program, the clinician summarizes the conditions or medications that may affect them.
  • Strength and mobility are marked by age group: 0 to 2, 2 to 6, 7 to 12 and 12 to 18 years, for people working or volunteering with children.
  • The current version is OCC 1204, revised 02/2023, and the form states all previous editions are obsolete.
