Maryland OCC 1215 Health Inventory (Child Care)

Before a child attends a Maryland licensed, registered or approved child care program or nursery school, the family must provide a physical examination on the state-designated form, immunization evidence and, for children under six, blood lead results. The Health Inventory carries the parent's history and the health care provider's Part II assessment that caregivers use to plan for the child.

Who completes it

The parent or guardian completes Part I and signs permission for the practitioner to complete Part II. A physician or nurse practitioner completes Part II, the child health assessment, or a copy of the child's physical exam is attached.

Submitting and payment

How to submit it
The completed form is returned to the child care provider through the parent before the child attends.
Who pays
The form sets no fee; it directs families without health insurance or a health care provider to their local health department.

Good to know

  • Part II asks about diagnosed conditions, specialist care and any condition that may require emergency action, which the provider also describes on the emergency card.
  • Findings are marked by body system and health area, with measurements for TB screening if indicated, blood pressure, height, weight, BMI percentile and developmental screening.
  • Immunizations go on MDH 896 or another official record and lead results on MDH 4620; medications at child care need the appropriate OCC 1216 form.
  • The current version, revised February 2023, states that all previous editions are obsolete, so older Health Inventory copies should be replaced.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a physical exam printout replace Part II of the Maryland Health Inventory?

Yes. The instructions say Part II must be completed by a physician or nurse practitioner, or a copy of the child's physical examination must be attached to the form. The parent still completes and signs Part I.

What lead testing does OCC 1215 expect?

Children younger than six in child care must have blood lead tests at 12 and 24 months. Two tests are required if the first was before 24 months; if the first test was after 24 months, one is enough. Results go on MDH 4620 or another official document.

Are exemptions allowed from the Maryland child care physical?

The form says exemptions from the physical exam, immunizations and blood lead testing are permitted for bona fide religious beliefs, with the lead certificate still signed by a practitioner after a questionnaire. Immunization exemptions are also allowed when a physician, nurse practitioner or health department official certifies a medical reason.
