Maryland MDH-4758 Youth Camp Medication Authorization

Maryland youth camps may only give a camper medication, or let the camper self-administer it, when this form is completed in full. Clinicians see it before camp season: the prescriber writes the order for each medication, the parent consents, and the camp keeps the form so staff know the dose, timing, side effects and whether the child may carry emergency medicine.

Who completes it

The authorized prescriber completes Section I, the prescriber's authorization, and signs Section III if self-administration is approved. The parent or guardian completes Section II and co-signs any self-carry approval.

Submitting and payment

How to submit it
The completed form goes back to the camp with the parent, and an authorized individual must hand the medication to an adult staff member.
Who pays
The form sets no fee and does not address payment for completing the order.

Good to know

  • A new form is required at the start of each camp season, for each medication, and each time the dose or time of administration changes.
  • The authorization runs for the year in which the prescriber dates it, unless narrower dates are entered, and may not exceed one year.
  • The prescriber line takes an original signature or signature stamp only, and the form states the parent or guardian cannot sign there.
  • Self-carry is limited to emergency medications such as inhalers and epinephrine, needs both prescriber and parent consent, and camps are not required to permit it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does a Maryland camp need a new MDH-4758 every summer?

Yes. The form says a new medication administration form must be completed at the beginning of each camp season, for each medication, and each time there is a change in dosage or time of administration. The authorization may not exceed one year.

Does MDH-4758 cover over-the-counter medicine?

Yes. Nonprescription medication, which the form says includes vitamins, homeopathic and herbal medicines, must be in the original container with instructions for use. Prescription medication must be in a container labeled by the pharmacist or prescriber.

Can a camper carry an inhaler or epinephrine under MDH-4758?

Only if Section III is completed. Both the prescriber and the parent sign to approve self-administration and check whether the child may self-carry emergency medication. Self-carry is limited to emergency medicines, and the camp operator may still decline to permit it.
