Iowa 470-0021 Certificate of Vision Screening

Iowa requires parents to have children screened for vision impairment at least once before enrolling in kindergarten and again before third grade in a public or accredited nonpublic elementary school. Form 470-0021 is the proof the school receives. The screening must fall between one year before and six months after enrollment in those grades.

Who completes it

The screening provider completes the screening section with date, pass or fail, method, acuity if available and referral, then signs with credentials; parents may attach a copy of results instead. An eye care professional completes the page 2 Eye Exam Section after a comprehensive exam.

Submitting and payment

How to submit it
Return the completed form to the child's school; the eye exam side goes to the school nurse or teacher.

Good to know

  • Testing method is marked as vision screening, photo screening or other; visual acuity with and without correction is recorded if available.
  • A comprehensive eye exam can satisfy the requirement instead of a screening, documented on the second side under Iowa Code 280.7A.
  • The source of screening can be the provider's office, or the school when the school nurse did the screening.
  • The current edition is 470-0021 (02/24); Iowa's dental screening for kindergarten and ninth grade is a separate certificate.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is the Iowa vision screening valid?

It must be done no earlier than one year before the date of enrollment in kindergarten or third grade and no later than six months after the child's enrollment in that grade. Screenings outside that window do not meet the requirement.

Can an eye exam replace the Iowa vision screening?

Yes. The form says the requirement can be met by a screening or by a comprehensive eye exam. The eye care professional completes the Eye Exam Section on page 2, including acuity, eye health, vision analysis and correction recommendations, and signs and dates it.
