Michigan MSA-6544-B Practitioner Prior Authorization

Michigan Medicaid's Program Review Division uses MSA-6544-B for prior authorization of practitioner services, including out-of-state care, for fee-for-service Medicaid and Children's Special Health Care Services (CSHCS) beneficiaries. Practitioners need it only when faxing a request: entering the request in the CHAMPS Provider Portal with the Supplemental Information section completed makes the form unnecessary. Standard reviews are decided within 7 calendar days.

Who completes it

The requesting provider's office completes boxes 1 to 29, including the referring or ordering practitioner (physician, PA or APRN), beneficiary details from the mihealth card, HCPCS codes, quantities, diagnoses and the documentation checklist. The provider or representative prints, signs and dates the certification.

Submitting and payment

How to submit it
MDHHS strongly encourages entering the request in CHAMPS by direct data entry and uploading documents there; otherwise fax this form to the Program Review Division. Mailing is discouraged, and Medicaid Health Plan members go to their plan.
Who pays
Authorization does not guarantee beneficiary eligibility or payment, and the provider is responsible for verifying eligibility.

Good to know

  • Expedited review is for services needed within 10 calendar days. Answer questions 16 to 20, and make sure the attached documentation supports the explanation, or the request is processed as standard.
  • Out-of-state requests need a letter of medical necessity explaining why care cannot be given in Michigan, which in-state services were exhausted, and the plan to transition care back.
  • Photos are required for all cosmetic and reconstructive surgeries. For injectable drugs, enter billing units; for CSHCS, give the qualifying diagnosis.
  • Extension requests must be submitted within 15 days before the authorization period ends. Illegible forms are returned, and editions before 3/2026 are obsolete.
