Michigan MSA-1653-B Special Services PA Request

Michigan Medicaid requires prior authorization through the Program Review Division for certain medical equipment and services, and MSA-1653-B is the request suppliers fax for them. DME and medical suppliers, orthotists, prosthetists, hearing aid dealers, audiologists and cochlear manufacturers use it for fee-for-service Medicaid and CSHCS beneficiaries. The ordering practitioner is named on the form, and their records support the request.

Who completes it

The requesting Medicaid-enrolled supplier completes boxes 1 to 29 and signs the provider certification. The referring or ordering physician, physician assistant or advanced practice registered nurse is identified by name, NPI and address in boxes 6 to 8, and their order and clinical documentation go with the request.

Submitting and payment

How to submit it
MDHHS strongly encourages entering the request in the CHAMPS Provider Portal, where this form is not needed and documents are uploaded to the request. The form is required only for faxed requests, and mailing is discouraged.
Who pays
Approved items are paid by Medicaid or CSHCS when billed, but authorization does not guarantee eligibility or payment, and the supplier is responsible for verifying eligibility.

Good to know

  • Standard requests are decided within 7 calendar days. Expedited review is for items needed within 10 calendar days and requires answers to questions 16 to 20 backed by the attached documents.
  • Box 14 needs the ICD diagnosis codes tied to the item; for CSHCS-covered services, give the beneficiary's CSHCS qualifying diagnosis.
  • Requests to extend an authorization period must be made within 15 days before it ends. Illegible forms and missing certification signatures are returned for resubmission.
  • Wheelchairs and complex seating use MSA-1653-D with the MSA-1656 evaluation instead, and enteral therapy requests pair with BPHASA-2401, the Medical Justification for Enteral Therapy.

Frequently asked questions

Is MSA-1653-B required if the PA is entered in CHAMPS?

No. The form states it is not required when the request is submitted by direct data entry in the CHAMPS Provider Portal. Supporting clinical documents are uploaded to the request there, each showing the beneficiary's name and mihealth card number. The form is required only for faxed submissions to the Program Review Division.

Does the prescriber sign Michigan form MSA-1653-B?

No. The supplier, or its representative, prints their name, signs and dates the provider certification, and requests without it are returned. The referring or ordering physician, physician assistant or advanced practice registered nurse is listed with name, NPI and address, and their order and medical records support the medical need.

How long does a Michigan Medicaid DME prior authorization take?

The instructions say standard reviews are determined within 7 calendar days. Expedited review applies when the item must be provided within 10 calendar days for medical necessity; if a request does not meet the expedited criteria, it is processed as standard and the supplier is notified in CHAMPS.
