Michigan MSA-181 Home Health Aide Prior Approval

Michigan Medicaid home health agencies use this typed request to get prior authorization for home health aide visits, billed as G0156, for up to 90 days at a time. Beyond the agency's details, it records the beneficiary's diagnoses, medications, cognition, OASIS function scores and the aide tasks requested. A practitioner signs that they examined the patient and that aide services are medically necessary.

Who completes it

The home health agency completes items 2 to 37. Three signatures are required: the patient or parent or guardian (item 38), the physician, nurse practitioner, clinical nurse specialist or physician assistant (item 39), and the supervising RN, PT, OT or speech-language pathologist (item 40).

Submitting and payment

How to submit it
Mail or fax the typed form with the most recent plan of care to the MDHHS Program Review Division. A copy of the OASIS goes with the initial request and annually after that.
Who pays
Approved visits are paid by Michigan Medicaid when the agency bills them, but approval does not guarantee beneficiary eligibility or payment.

Good to know

  • MDHHS requires the form to be typewritten and does not accept handwritten ones; a fill-in Word version is posted on its Medicaid provider forms page.
  • Each authorization covers no more than 90 days, so ongoing care needs a new request marked Continuing, with prior visit counts and dates.
  • Item 37 must list every other service in the home, such as private duty nursing, therapies, Home Help or hospice, with frequency and payer; omissions can lead to recoupment.
  • A hospital discharge planner or physician may start the request, in consultation with the beneficiary and the agency that will take over care.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
