Michigan BPHASA-2401: Enteral Therapy Justification

Michigan Medicaid requires the BPHASA-2401 to justify enteral formula, thickener or Relizorb for a beneficiary. The ordering practitioner documents the diagnosis and history behind the need, current height, weight and BMI, the weight trend, other coverage, and for up to three products the route, diet, daily caloric requirement and calories from formula, plus less costly alternatives already tried.

Who completes it

The ordering practitioner or designated clinical staff, such as a dietitian or registered nurse, completes all clinical fields, and the practitioner signs with specialty. The DME supplier may enter only the beneficiary ID, name, date of birth, therapy type and formula requested.

Submitting and payment

How to submit it
The DME supplier submits it to MDHHS; if faxed, it must accompany a completed MSA-1653-B that lists all requested enteral procedure codes. The form must be typewritten.
Who pays
Completion is voluntary but required when payment from the Medicaid program is sought for the enteral products.

Good to know

  • Height and weight must be current within 30 days of the request; for children under 3, give a weight-for-height ratio instead of BMI.
  • For a specialty formula, state the medical contraindication to standard, less costly options, and list economic alternatives tried, such as high-calorie shakes, OTC supplements or blended foods.
  • The 7-2024 version makes MSA-2401 and earlier BPHASA-2401 editions obsolete, so older copies should not be used.
  • Note any other insurance or program already covering formula for this beneficiary, which MDHHS uses for coordination of care.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can fill out the Michigan enteral therapy justification?

The ordering practitioner or the practitioner's designated clinical staff, such as a dietitian, registered nurse or other appropriate hospital clinical staff, completes the clinical fields, and the practitioner signs. The DME supplier may complete only the identifying fields, therapy type and formula requested.

Can one BPHASA-2401 cover more than one formula?

Yes. The form has space for up to three enteral products, each with therapy type (formula, thickener or Relizorb), oral or tube route, formula name, diet type, total daily caloric requirement, calories per day from the formula, and the alternatives tried.

Must BPHASA-2401 be typed?

Yes. The form states it must be typewritten, so complete the PDF on a computer. If it is faxed, it must go with a completed MSA-1653-B that lists every enteral therapy procedure code being requested.
