Michigan MSA-1656: Complex Seating Evaluation

Michigan Medicaid requires this evaluation for new or replacement wheelchairs, power mobility devices, seating systems, strollers, standers, gait trainers and children's positioning chairs. It travels with the MSA-1653-D prior approval request and the matching addendum, and must be submitted within 90 days of the evaluation date. Incomplete forms are returned to the evaluator.

Who completes it

A physical or occupational therapist, physiatrist or rehabilitation RN with at least two years of rehabilitation seating experience, not affiliated with the supplier, completes Sections 1 to 11; a PTA or OTA may not sign. For nursing facility residents, Section 10 is completed by the Director of Nursing, administrator or ordering physician.

Submitting and payment

How to submit it
For community beneficiaries, the evaluation, addendum and MSA-1653-D go to the ordering physician for review, and for residents back to the nursing facility; the set then goes to the MDHHS Program Review Division within 90 days of the evaluation.

Good to know

  • Addendum A covers manual wheelchairs with accessories, power mobility devices and seating systems; Addendum B covers strollers, standers, gait trainers and children's positioning chairs. A missing addendum delays authorization.
  • Section 6, the mobility assessment, is mandatory for every request. Visual perception, problem solving, comprehension and the hand muscle test are needed only for power mobility items.
  • Section 7 uses the Modified Ashworth Scale and manual muscle testing. If the beneficiary cannot be tested because of cognition or age, findings may come from observation, not self-report.
  • The evaluator attests to having no financial arrangement with the equipment supplier and that the item is the most economical alternative meeting basic medical and functional needs.
