Utah Medical Immunization Exemption: Provider Notice

Utah grants a medical exemption from school and early childhood immunization requirements when the parent gives the school a vaccination exemption form together with a written notice from a licensed health care provider. The notice states that, because of the student's physical condition, administering the vaccine would endanger the student's life or health. The provider writes the notice; there is no state provider form.

Who completes it

A licensed health care provider writes and signs the notice; the parent or guardian completes the vaccination exemption form and submits both.

Submitting and payment

How to submit it
The parent gives the provider's notice and the exemption form to the school or early childhood program, which keeps them in the student's permanent record.

Good to know

  • The exemption stays valid as long as the student remains enrolled in Utah's public education system and transfers with the student's records.
  • Personal and religious exemptions need no provider notice: parents complete an online education module, about 15 minutes in English or Spanish, or an in-person local health department consultation.
  • A child who is behind on vaccines can enroll conditionally and attend for 30 days while the family gathers records or updates immunizations.
  • The rules come from Utah Code 53G-9-303, as amended in the 2025 General Session and effective May 7, 2025.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What must a Utah medical exemption notice say?

The parent submits it to the school with a completed vaccination exemption form.

How long does a Utah medical exemption last?

The exemption form is valid for as long as the student remains enrolled in Utah's public education system. It becomes part of the student's permanent school record and carries over if the student transfers to another school.
