Nebraska Medical Exemption Form for School Vaccines

Nebraska exempts a student from school or child care immunization requirements when a medical provider states that a required vaccine would be injurious to the student or to someone in the student's household. This one-page form, revised May 2023, lets the provider check each disease the student will not be immunized against, choose whose health is at risk, add comments and sign.

Who completes it

A physician, physician assistant or APRN nurse practitioner completes and signs the whole form. Other exemptions, such as the religious affidavit, are signed by the parent or legally authorized representative.

Submitting and payment

How to submit it
Give the signed form to the family for the school or licensed child care program, which keeps it and reports exemptions in Nebraska's annual school or child care immunization survey.

Good to know

  • Check every disease for which a vaccine has not been given; the student still needs documentation of vaccination against all other required diseases.
  • The exemption can rest on harm to a member of the student's household or family, not only to the student, and the form has a box for each.
  • Diseases listed: COVID-19, Hib, hepatitis B, MMR, pneumococcal (PCV13), polio, tetanus, diphtheria and pertussis, and varicella.
  • The English PDF is a scanned image, so it is printed and completed by hand. Children with exemptions may be excluded during an outbreak.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
