Arizona ADHS Medical Exemption Form for School Vaccines

Arizona schools, preschools and child care facilities must keep this ADHS form on file to exempt a child from immunization requirements for medical reasons. It documents either that the child's health may be adversely affected, temporarily or permanently, by one or more required doses, or that the child has lab evidence of immunity or a documented history of disease.

Who completes it

A licensed physician or registered nurse practitioner prints their name, signs, dates, lists each exempted vaccine with its reason, and marks the exemption permanent or temporary with an end date. The parent or guardian signs an acknowledgement section below.

Submitting and payment

How to submit it
Give the signed form to the parent, who gives it to the school, preschool or child care facility to retain.
Who pays
ADHS posts the form for free download; it carries no fee or billing code.

Good to know

  • Lab results must be attached when claiming immunity to measles, rubella or varicella; documented history or lab evidence suffices for other diseases.
  • The parent acknowledges that during a declared outbreak the child may be kept out of child care or school until the risk period ends, which may be 3 weeks or longer.
  • The two-page PDF has English on page 1 and Spanish on page 2, last reviewed July 2026.
  • Authority is ARS 15-873 and AAC R9-5-305 and R9-6-706. Religious and personal belief exemption forms are separate and signed by parents only.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
