Maine School Vaccine Medical Exemption Statement

With religious and philosophical exemptions repealed for Maine schools on September 1, 2021, an unvaccinated child can attend only under a medical exemption, a one-time 90-day catch-up assurance or a narrow IEP exception. There is no required medical exemption form: the child's primary care provider writes it, and the Maine CDC Immunization Program cannot issue exemptions.

Who completes it

The child's primary care provider, who must be a licensed physician, nurse practitioner or physician assistant, writes the whole exemption. A naturopathic doctor cannot, and an out-of-state provider can only if also licensed in Maine at the time.

Submitting and payment

How to submit it
The family gives the written exemption to the school, which keeps a record of it showing the date it was accepted and the date it expires.

Good to know

  • The state's school immunization FAQ points to a sample medical exemption form on the Maine Chapter of the American Academy of Pediatrics website; using it is optional.
  • A child on a catch-up schedule may attend under a temporary medical exemption from a licensed physician, NP or PA, or under the parent's 90-day written assurance.
  • During an outbreak, a public health official can exclude children who are not immunized or immune, for example 15 days after the last measles case or 21 days for varicella.
  • Child care facilities follow separate immunization standards. Home-schooled students who join no school activities are not covered, but virtual schools are.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there an official Maine school medical exemption form?

No. The state FAQ says no form is required for a medical vaccination exemption and points to a sample form on the Maine Chapter of the American Academy of Pediatrics website. The exemption must come from the child's primary care provider, who must be a licensed physician, NP or PA.

Can Maine CDC grant a vaccine medical exemption?

No. Maine CDC states that all medical exemptions must be issued by the patient's primary care provider and that its Immunization Program cannot issue them. Questions about the school immunization law itself go to the Maine Department of Education.

What is the IEP exception in Maine's school vaccine law?

A student with an active IEP on September 1, 2021 who already had a philosophical or religious exemption can keep it, if the parent provides a statement from a licensed physician, NP or physician associate confirming they discussed the risks and benefits of immunizing. It lasts until the student graduates, ages out or leaves school.
