Missouri Imm.P.14: Immunizations in Progress Form

Missouri children who are behind on vaccines required for school or child care may keep attending only while a series is under way and the next dose is booked. This certificate, issued under sections 167.181 and 210.003 RSMo, records the vaccines given on a visit date and the scheduled return date, and is filed with the school or child care so staff can track the catch-up.

Who completes it

A physician, public health nurse or designee checks each vaccine given that day, enters the date given and the date the child is scheduled to return, then prints their name and signs.

Submitting and payment

How to submit it
The signed form is filed with the school administrator or child care center. After the scheduled appointment, an updated immunization record must be given to the facility.

Good to know

  • If the scheduled appointment is not kept, the child is no longer considered in progress, becomes non-compliant and cannot attend child care or school.
  • The child stays in compliance only while remaining doses are given at the correct intervals under ACIP recommendations, so the return date should match the next valid dose.
  • Vaccines that can be checked include diphtheria, pertussis, tetanus, hepatitis B, Hib, MMR, pneumococcal, polio and varicella, with an Other line for anything else.
  • During an outbreak or suspected outbreak at the facility, children who are not fully immunized or lack lab evidence of immunity may be kept out until the local health authority ends it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Missouri Immunizations in Progress form?

A physician, a public health nurse or a designee signs and dates it. The signer checks each vaccine given at the visit, writes the date given, and enters the date the child is scheduled to return for the next dose, so the school or child care knows when to expect an updated record.

What happens if a child misses the next vaccine appointment in Missouri?

Missouri's guidance for families says the appointment must be kept and an updated immunization record given to the school or child care. If it is missed, the child is no longer in progress, is non-compliant, and cannot attend child care or school until the required immunizations are up to date.

Is the Missouri Immunizations in Progress form available in Spanish?

Yes. The Department of Health and Senior Services posts the form in English and Spanish. Children with a medical reason not to receive a vaccine need a different document, the Imm.P.12 Medical Immunization Exemption, rather than the in-progress form.
