Missouri Medical Immunization Exemption (Imm.P.12)

Missouri's immunization laws (sections 167.181 and 210.003 RSMo) let a student be exempted from required vaccines on signed certification by a licensed MD or DO, or their designee. Imm.P.12 records that the immunization would endanger the child's life or health or is otherwise contraindicated, or that the child has documented disease or lab evidence of immunity.

Who completes it

A licensed doctor of medicine or doctor of osteopathy, or the physician's designee, checks the reason and the exempted vaccines, then prints a name and signs.

Submitting and payment

How to submit it
Place it on file with the school immunization health record or the child care facility.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
