New Jersey IMM-53: Medical Exemption Request

New Jersey uses this standard form to exempt a student from school, preschool or child care immunization requirements on medical grounds. The provider marks each vaccine, chooses a temporary exemption with an end date or a permanent one, describes the contraindication or precaution, and signs an attestation that it matches ACIP or AAP Red Book guidance. The October 2026 edition runs three fillable pages.

Who completes it

A physician (MD or DO) or advanced practice nurse licensed in any US jurisdiction completes the whole form, including student details, the vaccine table and the signed attestation with NPI, license number and state of licensure.

Submitting and payment

How to submit it
Give a copy to the person requesting the exemption or send it directly to the school, preschool or child care center, and keep a copy for your records.

Good to know

  • All fields are required, and the state recommends the latest version of Adobe Reader for completing the certified fillable PDF.
  • Moderate or severe acute illness is a precaution for all vaccines, but New Jersey says medical exemptions should not be submitted for it because such illnesses are short-lived.
  • The attestation warns that supporting medical documentation may be requested and that misrepresentation can be referred to the State Board of Medical Examiners or another licensing agency.
  • The vaccine table includes inactivated, live attenuated and recombinant influenza, MenACWY, MenB and PCV13, PCV15 or PCV20 alongside the usual school-entry vaccines.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
