Pennsylvania H502.320 Medical Certificate (School)

Pennsylvania students missing required vaccine doses can attend school when their provider documents a plan to finish the series. This red and white card lists each remaining dose of DTaP or Td, Tdap, polio, hepatitis B, MMR, varicella and meningococcal vaccine with the date it will be given. It goes to the school nurse with the record of vaccines already received.

Who completes it

A physician, certified registered nurse practitioner, physician assistant or local health department completes the schedule and signs, circling the signer's title. The student's name, birthdate, address, parent and grade go at the top.

Submitting and payment

How to submit it
Give the signed certificate to the family for the school nurse, with the EHR printout of vaccines already given attached.

Good to know

  • Measles and rubella serology, with date and titer, and the date of physician-diagnosed mumps can be recorded on the card.
  • Pennsylvania's current school immunization regulations took effect at the start of the 2017-18 school year; this card carries a 03/17 revision date.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Pennsylvania school Medical Certificate?

The signature line lets the signer circle physician, certified registered nurse practitioner, physician assistant or local health department. The signer lists the date each remaining dose will be given so the school can follow the student's plan to complete the required vaccines.

What must be attached to the H502.320 Medical Certificate?

The card says to attach the EHR record of vaccines already given. The certificate itself only lists future doses, so the attached record shows the school nurse which doses the student has already received and which remain.
