Georgia Certificate of Immunization Form 3231 (GRITS)

Georgia requires a Certificate of Immunization, Form 3231, on file for every child through 12th grade attending child care, pre-kindergarten, Head Start, nursery or school, public or private. The certificate is generated from GRITS, the Georgia Registry of Immunization Transactions and Services, and also serves as the record of a medical exemption.

Who completes it

A physician's office or public health department prints the certificate from GRITS. When a child has a medical exemption, the physician signs the medical exemption box and records the review date.

Submitting and payment

How to submit it
Print it from GRITS and give it to the parent for the school or child care facility.

Good to know

  • The medical exemption is recorded on Form 3231 itself, in a box carrying the physician's signature and a review date.
  • Religious objections use a parent-filed Affidavit of Religious Objection to Immunization, Form 2208, filed with the school or child care.
  • All 7th graders and new entrants in grades 8 to 12 need Tdap and meningococcal conjugate vaccine; 11th grade entrants need an MCV4 booster unless the first dose was at 16 or later.
  • Authority is O.C.G.A. 20-2-771 and DPH Rule 511-2-2. Form 3300 covers the separate vision, hearing, dental and nutrition screening.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Where do I get a blank Georgia Form 3231?

DPH does not post a blank. Form 3231 is generated from GRITS by a physician's office or public health department, using the child's registry record, and printed for the parent to take to the school or child care facility.

How is a medical exemption documented in Georgia?

On Form 3231 itself. The certificate includes a medical exemption box that the physician signs, with a review date. Religious objections are handled differently: the parent files Form 2208, the Affidavit of Religious Objection to Immunization, with the school or child care facility.
