Wisconsin F-04020L: Student Immunization Record

Wisconsin schools collect this record to measure compliance with the student immunization law (Wis. Stat. 252.04). Parents list vaccine dates and return it within 30 school days of admission. Clinicians sign in two places: to attest a reliable history of varicella disease or titer-proven immunity, and on the health waiver line when a student should not receive certain vaccines for health reasons.

Who completes it

The parent, guardian, legal custodian or adult student completes most of the form and signs Step 5. A physician, physician assistant or advanced practice nurse prescriber signs the varicella and titer attestation, and a physician signs the health waiver.

Submitting and payment

How to submit it
The family returns the completed record to the school within 30 school days of admission, with lab reports attached for any titer claims.

Good to know

  • DHS now requires a chickenpox diagnosis from a qualified provider (MD, DO, NP or PA); a parent's report of disease no longer meets the varicella requirement.
  • Students behind on doses must get second doses by the 90th school day after admission and third or fourth doses by the 30th school day of the next year.
  • Religious and personal conviction waivers are checked and signed by the parent; only the health waiver line calls for a physician's signature.
  • Waived or incompletely immunized students may be excluded during an outbreak. Child care programs use the separate Child Care Immunization Record, F-44192.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who signs the varicella history on Wisconsin's Student Immunization Record?

A physician, physician assistant or advanced practice nurse prescriber signs the attestation that the student has a reliable history of varicella disease, and the same signature line covers titers showing immunity to varicella, measles, mumps, rubella or hepatitis B, with lab reports attached.

When is the Wisconsin Student Immunization Record due?

State law requires public and private school students to present written evidence of immunization within 30 school days of admission. The parent returns this form to the school by then, either showing all requirements are met or checking the in-progress or waiver option.
