Wisconsin MV3746: Certificate of Substance Examination

WisDOT Medical Review uses this certificate when a driver's alcohol or drug use is part of a licensing review. The assessing counselor classifies use from irresponsible use through dependency or remission, notes drinking pattern and chronicity for alcohol, recommends treatment, and states whether the driver is complying. After review, WisDOT may require follow-up reports on the same form.

Who completes it

WisDOT fills in the driver and license details. The substance use counselor or other competent authority completes Sections A through C and signs with title and office details.

Submitting and payment

Frequently asked questions

Who completes Wisconsin MV3746?

A substance use counselor or other competent authority who assessed the driver completes the dependency findings, treatment recommendations and compliance question, then signs with their title and office information. WisDOT fills in applicant details such as license type, date issued and examiner badge number.

Will a driver need more than one MV3746?

Possibly. The form tells applicants that after the report is reviewed they may be required to file follow-up reports, and WisDOT sends the form at the required time. Section C records whether the applicant is currently compliant with recommended treatment and asks for an explanation if not.
