Wisconsin MV3030V: Certificate of Vision Examination

WisDOT requires this vision report when a driver's eyesight needs review for licensing, sometimes on a regular schedule. The specialist records Snellen acuity with and without correction, temporal field in degrees, progressive eye conditions and color recognition, then recommends restrictions and a possible DMV driving evaluation. The report is advisory; the Secretary of Transportation makes the licensing decision.

Who completes it

The applicant fills in identifying details and the license applied for. An MD, DO, OD, PA-C or APNP completes the specialist section and signs, noting whether they reviewed an attached Driver Condition or Behavior Report (MV3141).
