Wisconsin MV3030B: Physical Exam for S or P Endorsement

Wisconsin drivers holding or applying for a school bus (S) or passenger (P) endorsement need this physical examination report before WisDOT considers licensing under Trans 112. It combines a required vision section with numerical acuity, temporal field and color recognition, an applicant health history, and a clinician review of conditions such as seizures, diabetes, sleep disorders and heart disease.

Who completes it

The applicant completes Section A for both S and P endorsements and signs a release to the employer, school board and WisDOT. The health care professional completes the vision section and Section B for S endorsement applicants, and signs with a license number.

Submitting and payment

How to submit it
Return the completed report to WisDOT Medical Review by mail, fax or email; incomplete forms are returned for completion.
Who pays
Any charges for the medical or vision examinations and for completing the form are the applicant's responsibility.

Good to know

Frequently asked questions

Who can sign Wisconsin MV3030B?

The examiner certifies they are licensed to practice as an MD, DO, PA-C, APNP or chiropractor (DC). Chiropractors must add their National Registry number. If a different examining authority performed the vision portion, that person signs the vision section separately with their license number and date.

What vision questions are on MV3030B?

The vision section requires numerical acuity readings for each eye, uncorrected and corrected. It asks whether the temporal field is at least 70 degrees from center in each eye, whether the applicant can recognize red, amber and green, and whether corrective lenses are required when driving.
