Wisconsin MV3644: Medical Examination Report

WisDOT Medical Review sends this four-page report when it needs a clinician's opinion on whether a person is medically safe to drive. The reason for the request determines which sections apply: general medical, mental or emotional, neurological, endocrine, cardiovascular or pulmonary. Section G asks whether the person is safe to drive a car, commercial vehicle or bus, and what restrictions or re-examination to require.

Who completes it

WisDOT fills in the applicant details and the reason it is requesting information. An MD, DO, PA-C or APNP completes Sections A and G for every applicant plus the condition-specific sections, and signs with specialty and license number.

Submitting and payment

How to submit it
Return the report to WisDOT Medical Review by mail, fax or email; incomplete forms are rejected.

Good to know

  • Checking No to whether the person is medically safe to operate a car or motorcycle results in automatic cancellation of the driver license.
  • For seizures, the exam must be at least 60 days after the episode, and a seizure within the past 90 days makes the patient ineligible to hold a license.
  • The endocrine section asks for a hemoglobin A1C, the last three fasting glucose readings (home results accepted) and any hypoglycemic reactions needing assistance.
  • For a patient with an ICD, Section E records whether an electrophysiologist has cleared them for car, commercial or bus driving.
