Georgia DDS-MR-MA: Mechanical Aid Form

When a Georgia driver can drive safely with adaptations, their healthcare provider uses this one-page form to tell DDS which devices or restrictions are needed. Options include hand controls, left accelerator, left and right exterior mirrors, automatic transmission, power brakes or steering, no highway, daylight only, corrective or bioptic lenses and prosthetic aids, with a reason for each.

Who completes it

The driver completes the Driver/Patient Section and signs. A healthcare provider, such as a physician or occupational therapist, selects the devices and restrictions, explains them, and adds name, license number or specialty, contact details and signature.

Submitting and payment

How to submit it
The provider must submit it to the DDS Medical Review Unit by fax or email, as printed on the form.
Who pays
The form lists no fee.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

What happens after DDS receives the Mechanical Aid Form?
