Georgia DDS-MR-274OLCDL: CDL Online Renewal Vision

Georgia commercial license holders who renew online have to upload an eye doctor's vision certification with their renewal. On this one-page form the optometrist or ophthalmologist checks the driver against FMCSA's vision standards and selects whether the patient meets them without restriction, meets them with corrective or bioptic lens restrictions, or lacks sufficient vision to drive safely.

Who completes it

The driver completes and signs the Driver/Patient Section, authorizing the eye doctor. The optometrist or ophthalmologist completes the Optometrist/Ophthalmologist Section with name, medical license number and state, address, phone, exam date and signature.

Submitting and payment

How to submit it
The driver logs into their DDS online account and uploads the completed form, along with a copy of the eye examination results.
Who pays
The form lists no fee.
