Georgia DDS-MR 287: Medical Report for Driver Review

After Georgia DDS opens a medical review, it sends the driver this 4-page report with a letter listing the medical concerns. The physician evaluates those concerns, covering neurological and consciousness episodes, cardiovascular and respiratory disease, mental health, orthopedic limits, diabetes and medications, and states whether the driver can safely operate a motor vehicle and, for license holders, a commercial vehicle.

Who completes it

The patient completes and signs page 1 with personal details, license class, health history and an attestation. A licensed physician completes pages 2 to 4 and signs page 4; commercial license holders need a provider on the FMCSA National Registry of Certified Medical Examiners.

Submitting and payment

How to submit it
The medical provider mails or faxes all pages, with a copy of the DDS letter and a fax coversheet, directly to the DDS Medical Review Unit in Conyers.
Who pays
The form lists no fee.

Good to know

  • Questions 6 and 7, other conditions affecting safe driving and whether the patient can drive safely, require a Yes or No answer.
  • Checking the neurological or psychiatric box means an evaluation by a neurologist or neurosurgeon, or a psychiatrist or psychologist, must be attached.
  • Drivers who don't comply within 30 days receive a revocation notice; conflicting physician reports go to the DDS Medical Advisory Board.
  • Attach an EEG report if one was performed; the current edition is DDS-MR 287 (02/2026).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

What is the deadline for the Georgia DDS-MR 287?

DDS asks the driver to get the medical evaluation done after it sends the form and letter. If the driver doesn't comply within 30 days, DDS issues a notice that driving privileges will be revoked for medical reasons, and the driver may request a hearing within 15 days of receiving it.
