New Mexico MVD-10124: Driver Medical Report

New Mexico's Motor Vehicle Division uses this two-page report for issuing, renewing or continuing a Class D, E or M license, permit or provisional license when a medical condition is in question. The physician states whether the patient can drive safely and recommends restrictions and a renewal interval; the Medical Advisory Board may review it, and MVD makes the final decision.

Who completes it

The physician or other licensed health care provider completes the whole form: applicant information, Physician's Report items 1 to 10, and their name, license number, signature and date.

Submitting and payment

How to submit it
Mail it to the MVD Driver Services Bureau, or the driver takes it to an MVD field office, which faxes the original to the Medical Unit in Santa Fe.

Good to know

  • List only significant diagnoses that could affect safe driving; the instructions say not to list conditions such as thyroid disease, COPD or cancer unless they actually affect driving.
  • Check DENIAL under item 10 only if, from a medical standpoint only, the patient is not capable of safe and competent driving; otherwise choose 1 year or 4 years.
  • The form cannot be used for commercial Class A, B or C licenses; vision problems go on MVD-10125, the Vision Report.
  • Field offices will not process a form with missing information and return it for completion, so every item needs a legible answer.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can New Mexico's Medical Report be used for a CDL?

No. MVD-10124 is for Class D, E and M licenses, driving authorization cards, permits and provisional licenses. MVD's procedures state it cannot be used for commercial Class A, B or C licenses, and vision findings go on the separate MVD-10125 Vision Report.

Who reviews New Mexico's driver Medical Report?

The form says a physician or panel of physicians may review it and request more medical information, and its instructions name MVD's Medical Advisory Board, which may make recommendations. The final decision to accept or deny the application belongs to MVD. The report is confidential and may not be used as evidence in any trial.

How long is a New Mexico medical report valid?

MVD treats the report as valid from the date signed for the period the physician approved. Item 10 asks for that interval, 1 year or 4 years, and a physician who believes the patient cannot drive safely from a medical standpoint checks DENIAL instead.
