Hawaii DOT-H2058 Medical Report for Driver License

Hawaii's county examiners of drivers require this medical report when an applicant's physical or mental fitness to drive is in question. The examining doctor reports on loss of consciousness, physical impairments, cognitive or psychological conditions with MMSE or MoCA scores for dementia, substance use, vision and medications, then gives a safe-driving opinion and recommends a license validity period.

Who completes it

The applicant signs the release authorization and the notice on page 1, and the reason for the report must be filled in before the exam. A licensed medical doctor, or another competent authority acceptable to the examiner of drivers, completes the Physician's Report and certification.

Submitting and payment

How to submit it
The completed report must reach the county examiner of drivers within 30 calendar days. It may be forwarded to the Hawaii Medical Advisory Board, which reviews reports by case number only.
Who pays
The applicant is responsible for any professional fee for the examination.

  • Keep the applicant's signed authorization for release of medical information in your files, as the notice to the medical examiner directs.
