Georgia DDS 270: Request for Medical Review

Georgia's Department of Driver Services starts a medical review when it receives written concern about a driver's ability, and this form is the standard way to raise it. Doctors, law enforcement, courts, relatives and concerned citizens can file it, and DDS then evaluates the driver's medical history, physical health and other factors, noting that age alone can't decide the outcome.

Who completes it

The reporter fills in the driver's information and their own section: law enforcement, medical professional, or concerned citizen. A medical professional gives their profession, whether the concern is medical, vision or other, and the underlying condition or diagnosis, then signs under penalty of perjury.

Submitting and payment

How to submit it
Mail or fax it to the DDS Medical Review Unit in Conyers; the post office box and fax numbers are printed on the form.
Who pays
No fee is listed.

Good to know

  • Anonymous requests will not be considered; the reporter must give a name, phone number and mailing address.
  • Because of confidentiality, DDS cannot share its final determination with the person who requested the review.
  • After a request, DDS sends the driver medical forms such as the DDS-MR 287 Medical Report; not complying within 30 days leads to a revocation notice.
  • Law enforcement should attach related crash or incident reports; the current edition is marked DDS-270 6/25.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a doctor ask Georgia DDS to review a patient's driving?

Yes. The DDS 270 has a medical professional section where you give your name, profession and contact details, mark whether the concern is medical, vision or other, and state the underlying condition or diagnosis. You sign a declaration that the information is true and in the interest of public safety.

Will Georgia DDS tell me the outcome of a medical review request?

No. The form explains that, due to confidentiality, DDS cannot share a final determination with the requester. DDS evaluates the driver's medical history, physical health and other factors that could affect safe driving, and age alone cannot be the deciding factor.
