Michigan PCM 215a: Judicial Admission Certification

Michigan probate courts use the PCM 215a when a petition seeks judicial admission of a person with an intellectual disability to a facility under MCL 330.1516. A clinician meets it as one of the examiners asked to certify whether the person meets the criteria. The back page reports the person's condition, available alternatives to admission and the recommended living arrangement, so the court can weigh less restrictive options.

Who completes it

Up to two examining physicians or licensed psychologists complete the Certification of Examiners on the front, each in a separate block with the examination date. The person who evaluated the individual completes the Report on Petition for Judicial Admission on the back.

Submitting and payment

How to submit it
The certification and report are filed with the probate court in the judicial admission proceeding.

Good to know

  • Each examiner states whether the person has an intellectual disability, meets the criteria for treatment, and can reasonably be expected to seriously injure self or others, with supporting facts.
  • An examiner can also certify that the person requires immediate admission to a facility to prevent physical harm pending the hearing.
  • The report lists alternatives with their availability: residential placement, day activity programs, outpatient treatment, custody of a friend or relative, home care and private psychiatric inpatient care.
  • Use is mandatory, and earlier versions cannot be used after March 28, 2019; the current edition is (2/19).
