Michigan PCM 208a: Supplement on Return to Hospital

Michigan probate courts use the PCM 208a when an individual on an assisted outpatient or combined treatment order appeals a return from authorized leave to a hospital or facility (MCR 5.743a). A clinician meets it when the hospital needs to justify the return. Attached to the clinician's PCM 208 clinical certificate, it explains why the individual came back, the treatment plan, and alternatives the court could order instead.

Who completes it

A physician, psychiatrist or licensed psychologist completes the whole one-page supplement and signs under the penalties of perjury.

Submitting and payment

How to submit it
The supplement is filed with the probate court together with the examiner's PCM 208 clinical certificate in the appeal of the return.

Good to know

  • The supplement does not stand alone: it states that the examiner's PCM 208 clinical certificate is attached and builds on its findings.
  • Alternatives listed include day or night treatment, residential placement, custody of a friend or relative, private psychiatric inpatient care, assisted outpatient treatment, home care and day activity programs.
  • If none of the alternatives merits exploration, the examiner must check that box and state the reasons.
  • Use is mandatory, and versions approved before the (2/19) edition cannot be used after March 28, 2019.
