Michigan PC 630: Physician Report in Guardianship

Michigan probate courts use the PC 630 in guardianship cases for an alleged incapacitated individual under MCL 700.5304. A clinician meets it after the court appoints them (order PC 629) to examine the person. The one-page report describes the person's infirmities, how they interfere with decision-making, current medications and their behavioral effects, and the decision areas where the person cannot make informed decisions, so the court can decide on a guardian.

Who completes it

The licensed physician or mental health professional appointed by the probate court completes the whole report, including their specialty and the date they last examined the individual.

Submitting and payment

How to submit it
The report is filed with the probate court before the guardianship hearing; when the case is in a circuit court family division, the court name and county go in the upper left corner.

Good to know

  • The four decision areas to check are where to live, personal financial affairs, consenting to supportive services, and authorizing or refusing medical treatment.
  • The report must list all medications with dosages and describe the effect of each medication on the individual's behavior.
  • A recommended rehabilitation plan is attached separately, and further comments can go on an extra sheet; the form cites MCL 700.5304 and MCR 5.405.
