Michigan PCM 208: Clinical Certificate (Mental Health)

Michigan probate courts require the PCM 208 when an examiner reports whether an individual is a person requiring treatment under the Mental Health Code. A clinician meets it when examining someone named in a petition (PCM 201) or after a court-ordered examination. The examiner either transmits a clinical certificate supporting hospitalization, assisted outpatient treatment or both, or reports that a certificate is not warranted.

Who completes it

A psychiatrist, licensed psychologist or physician completes the whole two-page report and certificate and signs under the penalties of perjury, noting the time of signing.

Submitting and payment

How to submit it
The certificate is transmitted to the probate court, either attached to the petition (PCM 201) or filed after a court-ordered examination.

Good to know

  • Before any questions, the examiner must read the printed advisement to the individual and certify having done so, including that the examiner may have to tell the court what the individual says.
  • The report records where and when the exam took place, its start time and length in minutes, and whether the examiner reviewed records or consulted current treatment providers.
  • Describe specific actions, statements, demeanor and appearance, naming the source of anything not personally observed; for a discharge petition, explain why treatment is or is no longer needed.
  • The (8/25) edition is mandatory from October 12, 2025. Hospitalization requires two clinical certificates, at least one of them from a psychiatrist.

Frequently asked questions

What must the examiner read before a PCM 208 examination?

A printed statement explaining that the examiner is authorized by law to advise the court whether the person has a mental condition needing treatment, whether in a hospital or as outpatient care, and that the examiner may have to tell the court what they observe and hear.

What criteria does the PCM 208 clinical certificate use?

Mental illness plus one of three findings: a reasonable expectation of serious physical injury to self or others, inability to attend to basic physical needs, or judgment so impaired that unwillingness to follow treatment presents a substantial risk of significant harm. Each requires facts in support.

Who cannot sign a Michigan PCM 208?

The examiner certifies not being related by blood or marriage to the individual, or to anyone who has filed or is known to be planning to file a petition in the case. The signer must be a psychiatrist, licensed psychologist or physician authorized by law to certify the individual's mental condition.
