Kentucky AOC-737.1: Tim's Law AOT Evaluation

Kentucky district courts use the AOC-737.1 in assisted outpatient treatment cases under Tim's Law (KRS 202A.0815). A clinician meets it when a court order sets an evaluation of the respondent before the AOT hearing. The sworn certification states whether the respondent meets each statutory criterion for court-ordered outpatient treatment, with facts and supporting documentation, so the judge can decide whether to order AOT.

Who completes it

A qualified mental health professional under KRS 202A completes the whole two-page affidavit, checking their credential (physician, psychiatrist, psychologist, qualifying psychiatric nurse, social worker, therapist, counselor or physician assistant). A notary completes the sworn-statement block.

Submitting and payment

How to submit it
The form is filed with the district court immediately if possible, and in any event within 72 hours of receiving the order setting the evaluation, excluding weekends and holidays.
