Kentucky AOC-720B: ICF/ID Involuntary Admission Exam

Kentucky district courts use the AOC-720B when a petition seeks involuntary admission of a person with an intellectual disability to an intermediate care facility (ICF/ID) under KRS 202B. A clinician meets it after examining the respondent for the court case. The sworn certification gives the professional's opinion on intellectual disability, danger, benefit from ICF/ID treatment and least restrictive placement, with facts for each finding.

Who completes it

A qualified intellectual disabilities professional (physician, licensed psychologist, qualifying registered nurse, social worker, marriage and family therapist or professional counselor) or a licensed psychiatrist, psychologist or physician with special training and experience in intellectual disability completes the whole affidavit. The affiant also states whether they work in the community or for a state-operated ICF/ID.

Submitting and payment

How to submit it
The completed certification is sworn before a notary and filed with the district court hearing the involuntary admission case.

Good to know

  • Danger is defined on the form as substantial physical harm or its threat, including actions that deprive self, family or others of basic means of survival such as shelter, food or clothing.
  • The benefit question is specific: whether the respondent can reasonably benefit from treatment available in an ICF/ID, not treatment in general.
  • Least restrictive placement may include an institutional placement when it is the least confining setting that provides appropriate treatment or care consistent with accepted professional practice.
  • The current edition is Rev. 1-26, citing KRS 202B.100, 202B.120 and 202B.130; related forms in the case are AOC-710B, AOC-715B and AOC-730B.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Kentucky AOC-720B?

A qualified intellectual disabilities professional, such as a Kentucky-licensed physician, a psychologist licensed under KRS Chapter 319, or a nurse, social worker, therapist or counselor who meets the experience requirements printed on the form, or a licensed psychiatrist, psychologist or physician with special training and experience serving people with an intellectual disability.

What findings does the AOC-720B require?

Four checked findings with supporting facts: whether the respondent is intellectually disabled, whether they present a danger or threat of danger to self, family or others, whether they can reasonably benefit from ICF/ID treatment, and whether ICF/ID placement is the least restrictive alternative. A diagnostic impression follows.

Is the AOC-720B the same as the AOC-720A?

No. The AOC-720A covers involuntary psychiatric hospitalization of a person with mental illness under KRS 202A, while the AOC-720B covers involuntary admission of a person with an intellectual disability to an ICF/ID under KRS 202B, with criteria written for that setting.
