Curateur public Psychosocial Assessment Forms (QC)

These assessments give the court the social side of a protective measure: living environment, finances, legal proceedings, maltreatment, faculties to exercise civil rights and the views of the person and their relatives. The Curateur public posts four versions: opening a tutorship, homologating a protection mandate, temporary representation and reassessment of a tutorship. Each accompanies the physician's medical assessment.

Who completes it

A social worker or another person authorized under the Professional Code completes the whole form, after the mandatory training the Curateur public requires for its psychosocial evaluation and reassessment forms. Physicians complete the separate medical assessment, which must already have found the incapacity.

Submitting and payment

How to submit it
Under the tutorship version's instructions, the report is sent only if incapacity is found: the original goes to the competent person of the establishment when the Curateur public applies, or to the applicant who attested under oath when relatives apply, with a copy to the person concerned.
Who pays
The Curateur public's reference guide states the psychosocial assessment is covered by the health and social services network, and the institution pays when it mandates a professional in private practice.

Good to know

  • Choose the version that matches the recommendation: recommending homologation of a protection mandate requires the mandate-specific form, not the tutorship version.
  • The form is not a channel for reporting abuse: suspected maltreatment must be reported to the appropriate body, such as the CDPDJ or the Curateur public.
  • The person must be informed of the assessor's conclusions and given a chance to comment, and all interested relatives should be consulted, as Sections 10 and 11 record.
  • The assessor recommends a psychosocial reassessment time limit of up to five years, which may differ from the physician's recommended medical reassessment time limit.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can a physician complete the psychosocial assessment?

No. In this context, the psychosocial assessment is an activity reserved for social workers and persons authorized under the Professional Code. Assessments by other professionals, such as an occupational therapy functional assessment, can be attached when they were needed, but the physician's part is the separate medical assessment form.

What does the tutorship version ask the assessor to recommend?

Section 12 covers incapacity and the need for representation, tutorship to the person, to property or both, terms such as the right to vote and the need for a guardian, proposed and recommended tutors, any urgent provisional protective measure and the reassessment time limit.

Are the psychosocial assessment forms available in French?

Yes. Each version is posted in French and English on the Curateur public's forms page for the health and social services network, including Évaluation psychosociale dans le cadre de l'ouverture d'une tutelle au majeur and Réévaluation psychosociale dans le cadre d'une tutelle.
