Québec Tutorship Reassessment: Assessor's Notices

Québec's Curateur public publishes three short forms for the periodic reassessment of an adult under tutorship. The assessor completes a notice when the situation is unchanged and the tutorship should be maintained, or a report when only the reassessment time limit should change. When reassessments recommend modifying or lifting the tutorship, the institution's competent person completes a notice that goes to court with both reports.

Who completes it

The physician or psychosocial assessor who did the medical or psychosocial reassessment completes the maintenance notice or the time-limit report and gives their licence number. The notice recommending modification or lifting is completed by the establishment's competent person, such as a public institution's medical and professional services director, after obtaining the second assessor's report.

Submitting and payment

How to submit it
The maintenance notice and the time-limit report go to the tutor and the person under tutorship, and the tutor files the time-limit report with the court. The modification or lifting notice is filed with the court clerk with both reassessment reports.

Good to know

  • The maintenance notice confirms that the tutor met the obligation to have the person reassessed within the time limit set by the court or by law.
  • The time-limit report asks for the current and recommended reassessment intervals in years, a justification, and the relatives the court must notify.
  • If the tutorship is divided, copies go to the tutor to the person; for a tutorship to property only, they go to the tutor to property.
  • The person must receive both reassessment reports; withholding them must be exceptional under article 278.1 of the Civil Code of Québec, with the reason recorded.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Which Curateur public form does the assessor use after a tutorship reassessment?

If the situation is unchanged, the assessor uses the notice concluding that the tutorship should be maintained. If the tutorship should continue but the reassessment interval should change, the assessor uses the report on modifying the time limit. If modification or lifting is recommended, the assessor completes a reassessment report and the institution completes the notice.

Are the tutorship reassessment notices available in English?

Yes. The Curateur public posts English and French versions, for example the Assessor's opinion related to a reassessment concluding on the need to maintain the tutorship. Its page recommends reading the Aide-mémoire on tutorship reassessment before choosing or completing the forms.

What if the medical and psychosocial assessors disagree?

The notice recommending modification or lifting has an option stating that the two reports do not concur and that, after discussion, the assessors maintain their separate conclusions. The form notes that the court decides on the evidence and encourages assessors to share their conclusions for consistency.
