Medical Reassessment Related to a Tutorship (QC)

Every adult tutorship in Québec must be reassessed within the time limits the court sets. A physician uses this form when the person's situation has changed enough to modify the nature or terms of the tutorship, or to end it. With the psychosocial reassessment and a notice, the establishment files it at the courthouse, and if no one contests within 30 days the court issues its finding.

Who completes it

A physician who has examined the person completes the whole form, including the recommendation in Section 7 and the time limit in Section 8. A social worker completes the psychosocial reassessment, and if one evaluator recommends a change, the other must also produce a reassessment.

Submitting and payment

How to submit it
Send a copy to the competent person of the establishment, who files the reports at the courthouse, and a copy to the person concerned and their tutor; if a psychosocial reassessment already recommended a change, send it to the competent person whatever your conclusion.
Who pays
The RAMQ pays for medical reassessments, though billing terms vary, so the Curateur public refers physicians to their professional federation for details.

Good to know

  • Use this form only to modify or end the tutorship; if you conclude it should be maintained, or that only its time limit should change, separate Curateur public forms apply.
  • The reassessment time limit cannot exceed five years, or ten when the person's condition will clearly not change, and the next date counts from the latest judgment or finding.
  • Originals are not required for reassessment forms, and filings at the courthouse cannot be sent by email or fax.
  • The person under tutorship may ask to be reassessed at any time, not only at the date fixed in the judgment, under the Civil Code of Québec.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

What can the physician recommend on this form?

Section 7 offers four choices: maintain the nature of the tutorship, lift the tutorship to property, lift the tutorship to the person, or change it to a tutorship to both the person and property. You can also add details about the person's faculties, for example to support changes to the terms.

Who receives the medical reassessment?

The competent person of the establishment, for example the medical and professional services director of a public institution, plus the person concerned and their tutor. If the tutorship is divided, copies go to the tutor to the person, or to the tutor to property when the tutorship covers property only.

Is the medical reassessment form available in French?

Yes. The French edition is Réévaluation médicale dans le cadre d'une tutelle. Both language versions are posted on the Curateur public's forms page, with the psychosocial reassessment, the notice recommending modification or termination, and an aide-mémoire on tutorship reassessment.
