CNESST OSI 6074: Rehabilitation Before Consolidation

Before CNESST puts a rehabilitation measure in place for a worker whose injury is not yet consolidated, section 145.2 of the LATMP requires it to submit the measure to the treating health professional if it could affect the worker's health. CNESST generates this form for the file, describes each proposed measure, and asks whether you consider it appropriate to the worker's state of health.

Who completes it

CNESST fills in the worker's details and describes the proposed measures. The treating health professional (professionnel de la santé qui a charge) answers yes or no for each measure in section 2, chooses any preparatory measure in section 2.1, adds comments and signs section 4.

Submitting and payment

How to submit it
Return the signed form to CNESST through its Envoi de document electronic transmission service, by fax or by mail.
Who pays
When CNESST uses this form, the guide states the treating professional is paid under the agreement on remuneration for medico-administrative services (entente sur la rémunération des services médico-administratifs).

Good to know

  • Measures CNESST may propose include psychosocial intervention, vehicle, leisure equipment or workstation adaptation, retraining, job-search support, vocational evaluation and a gradual return to job tasks.
  • When CNESST proposes a gradual return to job tasks, section 2.1 lets you add a preparatory functional capacity evaluation or development, or tick that none is needed.
  • Functional capacity evaluation runs in blocks of up to 6 hours a day, to a maximum of 18 hours; development runs 3 to 6 hours a day, up to 160 hours.
  • The form is not mandatory: CNESST may also obtain your opinion in writing or by phone, and a measure proceeds once your opinion is favourable.

Why does CNESST need my approval before consolidation?

Section 145.2 of the LATMP requires CNESST, before granting or implementing a rehabilitation measure before an employment injury is consolidated, to submit it to the treating health professional, unless the measure has no effect on the worker's health.
