CNESST Rapport complémentaire (2349): 30-Day Reply

When a professional designated by CNESST or chosen by the employer contradicts the treating professional on diagnosis, consolidation, care, permanent impairment or functional limitations, CNESST or the employer sends this report so you can support your conclusions. Under sections 205.1 and 212.1 of the LATMP you have 30 days to reply, and CNESST may then refer the reports to the Bureau d'évaluation médicale (BEM).

Who completes it

CNESST or the employer completes sections 1 to 3: the worker, both professionals and the subjects on which your opinion is sought. The treating health professional completes section 4, Conclusions étayées, and signs to confirm they informed the worker of the report's content.

Submitting and payment

How to submit it
Send it to CNESST within 30 days of receiving the designated professional's report, attaching a reasoned consultation report (rapport de consultation motivé) if you consulted a colleague, and inform the worker of its content without delay.
Who pays
CNESST pays the report under RAMQ code 9909, printed beside the signature line.

Good to know

  • Answer every subject ticked in section 3; the CNESST guide says that if you do not respond, CNESST must have the worker evaluated by a member of the Bureau d'évaluation médicale.
  • For each subject, explain why you accept or reject the designated professional's conclusions; the five subjects mirror paragraphs 1 to 5 of section 212 of the LATMP.
  • A rapport de consultation motivé has no prescribed form: it is a colleague's opinion you request, if you judge it necessary, to support your report.
  • The paper version is a four-copy set: white for CNESST, yellow for the treating professional, pink for the designated professional and gold for the worker.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the deadline for a CNESST rapport complémentaire?

Thirty days from receiving the designated health professional's report. Sections 205.1 and 212.1 of the LATMP let the treating professional supply a supplementary report within that period to support their conclusions, with a reasoned consultation report if any, and require them to inform the worker of its content without delay.

What happens after I send the rapport complémentaire?

Under section 212.1, CNESST submits the reports, including your supplementary report, to the Bureau d'évaluation médicale when its own designated professional contradicted you. Under section 205.1, where the employer's designated professional disagreed, CNESST may submit them. The CNESST guide says a missing reply also leads to a BEM evaluation.

Can I use the rapport complémentaire posted on CNESST's website?

No. CNESST says the form on its website is provided for information only and that only the original form will be accepted. The original is sent to you by CNESST or by the employer, with sections 1 to 3 already completed and the subjects for your opinion ticked.
