CNESST IMCE Form 2096: Supplementary Medical Info

A CNESST médecin-conseil or dentiste-conseil uses this written request (information médicale complémentaire écrite, or IMCE) when they need clarification from the treating health professional on a worker's file. CNESST fills in the worker details, the nature of the request and your identity; you answer in section 4. Section 202 of the LATMP gives you 10 days from receipt to reply.

Who completes it

Only the treating health professional (professionnel de la santé qui a charge) completes section 4, Rapport du professionnel de la santé qui a charge du travailleur, and signs it. CNESST completes sections 1 to 3, and the requesting médecin-conseil signs the nature of the request.

Submitting and payment

How to submit it
Complete section 4 on the original form CNESST sends and return it within 10 days of receiving the request; CNESST says only the original form is accepted, and the version on its website is for information only.
Who pays
CNESST pays the reply under RAMQ code 09929, printed beside the signature line of section 4.
