Hawaii TB Form F: TB Clearance Form

Hawaii requires TB clearance under Hawaii Administrative Rules chapter 11-164.2 for schools, child care, food handlers and health care settings, and the Department of Health's TB Form F is the approved form for documenting it. The practitioner certifies, using the DOH TB Clearance Manual, that the person does not have TB disease. Separate sections cover school screening and health care facility screening.

Who completes it

A licensed practitioner (MD, DO, APRN, NP or PA) completes the whole form after the patient identifiers and signs it or applies a unique stamp, with facility details. The DOH TB program does not complete Form F; the patient's own provider does.

Submitting and payment

How to submit it
Give the completed form to the patient, who submits it to the school, child care facility or employer that requires clearance.

Good to know

  • For schools, child care and food handlers, clearance is a negative TB risk assessment, a negative TST or QFT, or a positive test with a negative chest X-ray.
  • Health care facility screening can require a 2-step TST or QFT; prior positive tests are handled with a symptom screen, risk assessment and recent chest X-ray.
  • DOH also posts TB Document G (risk assessment) and TB Document H (symptom screen), dated May 19, 2025, for providers to use alongside Form F.
  • Clearance gives reasonable assurance the person was free of TB disease at the exam, but implies no protection from future TB risk.
