Hawaii DHS 908 Pre-K Health Record Supplement

Children entering licensed child care or preschool in Hawaii need the Department of Education's Student's Health Record (Form 14), and DHS 908 is the Department of Human Services supplement that goes with it. It records head circumference, Hgb/Hct, lead, BMI and developmental screening results, plus medical conditions that may need a special care plan from the early childhood provider.

Who completes it

The physician, NP, APRN or PA (or clinic) completes boxes 1 to 10 and signs or stamps. The parent completes the consent in boxes 11 to 13, and the early childhood provider uses box 8 when a special care plan is needed.

Submitting and payment

How to submit it
Return it to the parent for the child care facility or preschool, together with the completed DOE Form 14.

Good to know

  • Mark each screening normal or abnormal, no concern or concern, or normal or counsel; any abnormal, concern or counsel result needs box 4 recommendations or follow-up.
  • Developmental screening tools listed are PEDS and ASQ, or the name of another tool; head circumference applies up to age 2 and BMI from age 2.
  • Mark 'None' or list allergies, medications, special diets, behavioral concerns and medical conditions, and mark Yes if the program should develop a special care plan.
  • Page 4 is a sample Special Care Plan for a Child with Allergy, with a physician signature line.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does Hawaii DHS 908 replace DOE Form 14?

No. DHS 908 is a supplement used together with the DOE Student's Health Record, Form 14. The child's physician is asked to complete both forms for preschool or child care entry, and DHS 908 adds early childhood screening results and special care details.

What is a special care plan on the DHS 908?

If the child has a medical condition and the early childhood provider should develop a special care plan, the physician marks Yes in box 6 next to the category, such as allergies or medications. The program then completes box 8. DHS offers sample special care plans, including one for allergy on page 4.
