Hawaii DHS 984 Medical Report for Child Care

Hawaii's Department of Human Services uses DHS 984 to evaluate whether a child care applicant, employee, substitute, volunteer or household member can care for children or have contact with them in a licensed facility. Part II records TB clearance, any physical disability or mental health history or limitation affecting caregiving, and the examiner's opinion on communicable disease and fitness.

Who completes it

The applicant, employee or household member completes Part I, including facility type, position and an authorization to release the report. A physician, PA, APRN or NP completes and signs Part II.

Submitting and payment

How to submit it
Return the completed form to the person examined or to the name and address listed in Part I, such as the child care facility or the DHS licensing worker.

Good to know

  • The TB section is completed only if other verification of TB clearance is not attached: a negative risk assessment and symptom screen, a negative TB test, or a negative chest X-ray after a positive test.
  • The form asks for every item to be completed or marked 'not applicable', with an explanation for any Yes on history or limitations.
  • Facility types include family child care homes, group child care centers and homes, infant and toddler centers, before and after school facilities and legally exempt homes.
  • The current edition is DHS 984 (REV 03/2026), posted as a fillable Word file; the forms page also links an older archived PDF edition.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who needs a Hawaii DHS 984 medical report?

Applicants, employees, substitutes, volunteers and household members in licensed child care settings, including family child care homes, group child care centers and homes, infant and toddler centers, and before and after school facilities. Legally exempt homes are also listed. The report helps DHS evaluate fitness to care for children.

Is a separate TB clearance needed with the DHS 984?

Not necessarily. Part II includes a TB clearance section to complete if other verification of TB clearance is not attached. The examiner checks a negative risk assessment and symptom screen, a negative TB test, or a negative chest X-ray after a positive test, with dates.
